Male fertility tests: semen analysis and hormone checks

At a glance

Semen analysis is a primary investigation in male fertility assessment. A doctor considers reproductive history and examination alongside the result. Hormone blood tests may help when symptoms, examination or semen findings suggest they are useful.[7]

What a semen analysis measures

The laboratory examines sperm concentration, movement and shape, with other measures where appropriate. RCPA describes semen analysis as a pivotal test in male fertility assessment. Your doctor explains the report in the context of your history rather than treating a single number as a complete answer.[7]

When hormone blood tests are useful

Healthy Male’s guidelines recommend hormonal evaluation when male-factor infertility is suspected, including abnormal semen parameters, small testes or symptoms of androgen deficiency. A testosterone result alone cannot replace a semen assessment. The purpose of each investigation should be explained before it is ordered.[8]

Prepare according to the laboratory instructions

Semen collection has specific instructions about preparation, the collection container and getting the sample to the laboratory. Ask the collection service for its written requirements before the appointment. Follow those instructions and tell the service if the sample was incomplete or delivery was delayed.[9]

Morning collection for relevant hormone tests

Where endocrine assessment is indicated, Healthy Male recommends morning blood collection, ideally fasting. Follow the requesting doctor and laboratory’s instructions for the actual tests. Tell the doctor about your medicines, supplements and any previous reports, so the history and results can be considered together.[8]

Start with an assessment

Discuss your reproductive history, concerns and family-planning goals with your GP. An examination may be needed even when part of the consultation happens by telehealth. Healthy Male’s guidelines describe history, physical examination and semen analysis in the initial evaluation. The doctor can decide whether a specialist referral is useful.[8]

Confirm the fertility-specific fee

Ask for the consultation, semen analysis, any hormone blood tests and the results appointment as separate items. The existing comparison records hormone-assessment fees; those amounts are not a quote for a fertility assessment or semen analysis. No fertility-specific complete fee is established from the checked Nexus service page. Ask the clinic for the relevant written quote.

Keep the result and follow-up plan together

Ask who will explain the report and whether another sample or appointment is recommended. Keep the original laboratory report and referral with your appointment notes. Check the price and inclusions before booking a repeat service. Hormone Test does not collect your results or determine fertility from a questionnaire.
